Abstract :
Questionnaires play the most important role in scientific papers and workplaces. Considering various applications, questionnaires may be designed in different shapes and types. In any case, it is critical to have a questionnaire with appropriate content and a pleasing design. There has been a lot of study done on the validity and reliability of questionnaires used in performance assessments of managerial levels, but users are not sure whether their data is valid or not, yet (Before, it was observed that respondents used steady responses due to boring shapes of the options in questionnaire). This matter has a direct effect on projects or processes you are going to do. This paper considers a questionnaire using in performance assessment of managerial levels in a large company, and presents a new form of questionnaire which combines 5-point Likert scale with competencies definitions. New questionnaire is able to solve some problems in assessment of performance such as invalid data and low return rate. Cronbach s alpha (reliability of this questionnaire) using SPSS software is 0.891.
