Abstract :
Breaking bad news (BBN) is one of the most challenging tasks for healthcare providers, traditionally considered the sole responsibility of physicians. However, evidence suggests that nurses—especially in emergency departments (EDs)—play a far more extensive role than formally defined in their job descriptions. This study aimed to investigate the actual role, challenges, and educational needs of emergency nurses in breaking bad news to patients. This cross-sectional study was conducted on 135 patients admitted to the emergency departments of teaching hospitals affiliated with Kurdistan University of Medical Sciences in 2021. Sampling was performed using a quota convenience method. Data were collected using a demographic information form, a researcher-made questionnaire assessing personnel performance (19 items, dichotomous responses), and a validated questionnaire assessing patients attitudes (19 items, 5-point Likert scale). Data analysis was performed using SPSS version 16, employing independent t-tests, ANOVA, Chi-square tests, and Spearman s correlation coefficient. A significance level of p < 0.05 was considered. The majority of patients (69.6%) had received bad news from nurses, while only 30.4% received it from physicians. The most common bad news delivered by nurses was abnormal laboratory results (25.9%). Overall personnel performance indicated that only 32% of the desirable conditions mentioned in the questionnaire were observed. The patients overall attitude score was 3.14 ± 0.74 out of 4, indicating a high preference for adherence to standard principles in BBN. A significant difference was observed between the performance of nurses and physicians (p = 0.004), with patients rating physicians performance higher. Additionally, a significant relationship was found between prior experience of receiving bad news and personnel performance (p = 0.007). Emergency nurses play a substantial role in breaking bad news, which is inconsistent with their formal job descriptions and received training. The performance of personnel is unsatisfactory compared to patients expectations. Developing clear guidelines, redefining the role of nurses, and providing targeted training in academic and in-service programs are essential to improve the quality of BBN and reduce its negative consequences.
